The goal for PHSC students with disabilities is to provide useful services that would contribute to successful learning environments, and provide complete accessibility that supports full inclusion. We strive to help those with disabilities reach their educational goals while unlocking their potential.
How to Request Accessibility Accommodations
- Step 1 - Schedule an Appointment

Contact Student Accessibility Services to set up an appointment. Visit the Student Accessibility Services Resources web page to view the business hours and contact information for the campus you plan to attend. Collapsible content 1.
- Step 2 - Submit the Accommodations Form

Complete the Accommodations Form. This form can also be completed in person with the Assistant Coordinator, or the Coordinator of Student Accessibility Services.
- Step 3 - Provide Documentation

Bring any documentation regarding your disabilities to the scheduled appointment. This may include:
- Individualized Education Plans (IEP)
- 504s
- psychological testing results, or
- other documents from a physician
Additional documentation may be required depending on the accommodations requested. Provisional or temporary accommodations may be accepted until further documentation is provided.
The paperwork and documentation (Steps 2 and 3 above) requesting an accommodation will take 3-5 business days to process. Accommodations will be sent to you through your PHSC email.*
